11. Financial (loan, credit, investments, etc) Records
12. Insurance Documents
13. Medical Records
14. Mlitary Records
15. Vehicle Records

You must provide proof from three different acceptable residency categories.

1. Original birth certificates issued by federal, state or provincial registrars within one year of your child’s birth date
2. Passports
3. Visas
4.  “Statement in Lieu of Acceptable Proof of Birth” issued by a District Administrator of Little League Baseball, Inc.

Not Acceptable as sole proof of birth are:

1. Baptismal Certificate
2. Certificate of Blessing
3. Certificate of Dedication
4. Certificate of Circumcision, etc.
5. Hospital Certificate; photocopied records.

Girls’ Softball Fees

• Ages 8-10 by 12/13/11 – $50;
•  Ages 11-12 by 12/31/11 – $55;
• Ages 13-16 by 12/31/11 – $60.
